Medication Management
Residents of traditional health care facilities are not the only ones that can enjoy the benefits of our consultant pharmacist service. Through our medication therapy management service our consultant pharmacists will meet with anyone face-to-face to discuss concerns about all facets of medication therapy. After a thorough review of all medications and medical conditions the patient, family, physicians, and the consultant pharmacist agree upon a medication therapy care plan. Based on goals approved by your physician our consultant pharmacists can help make recommendations to achieve those goals.
